Legal Terms (Please Read) I authorize Certa ProPainters, Ltd. to send a donation request form to the CertaPro Painters franchise listed below for $10 per Residential and Commercial painting project completed between October 1 - November 30, 2020. The number of painting projects completed between October 1- November 30, 2020 will be validated by WebRaags. Both exterior and interior painting projects will be included in the program.

I understand that I will be sent a donation request form between December 1^st – December 31^st, 2020. I understand the donation request form will be emailed to the email address provided below.

I understand that the donation will be taken out of my account with auto debit. I authorize Certa ProPainters, Ltd. to remove the funds from my account based on the amount listed on the donation request form.

I understand that the Breast Cancer Research Foundation (BCRF) will send a tax acknowledgement document to the address listed below. Certa ProPainters, Ltd. Is not responsible for the tax acknowledgement letter. I understand that it may take up to 120 days after my donation request is paid in full until I receive the tax acknowledgement letter from BCRF.
